Because it's not a staple anymore in the current gamestate. Just go look at the current tier 1 decklists. Almost no one mains it anymore because of all the Dracossacks, Spellbook of Wisdoms and Infestation Pandemics. Neutral

Meh, I still see more benefits from it since you won't always be vs'ing a deck that runs those.. And since you're playing Evilswarms just activate the infestation pandemic and then dark hole...

You don't get my point.
It's useless if the card(s) you want to destroy with it won't be destroyed anyway. You're just wasting a card.

How hard is it to use Ophion's effect, search off of Judgment Day or use Dracossack's effect? And how often does it happen?

Let me read you Evilswarm Ophion's Effect;

2 Level 4 "lswarm" monsters
While this card has Xyz Material, Level 5 or higher monsters cannot be Special Summoned. Once per turn: You can detach 1 Xyz Material from this card; add 1 "Infestation" Spell/Trap Card from your Deck to your hand.

I can detach one to add Infestation Pandemic so I can stop your dark hole, with this format it's better not to run Dark Hole in the main deck because cards like Infestation Pandemic and Spellbook of Wisdom and even Elemental Dragons can re-summon themselves the very next turn.(Even then you still face Draggosack and LaDD) It's better in this format to replace Dark Hole with a better card that can help stop these decks or help you out more with what the goal of your deck is. Because once you get to that goal it'll help you out more then wasting one card in your hand then getting OTK'd
